From Animorphs book 54: The Beginning, by K.A. Applegate.

SPOILERS

The battle that ends the war. Rachel infiltrates the Blade ship with Tom, then morphs grizzly to kill him. The other Animorphs and Visser One watch from the Pool ship's view screen. Tom morphs a cobra and bites Rachel while she fights his crew. She kills Tom and demorphs, then a Controller in polar bear morph kills Rachel.
